TriMix Injections for Erectile Dysfunction

According to the American Urologic Association, an intracavernous injection is the most effective non-surgical treatment for ED. Who Is a Candidate for TriMix Injections?

What Is the Appropriate TriMix Injection Dosage?

TriMix injections are a medication that is very customized to the patient’s needs. We recommend using a lower dose in the beginning to prevent prolonged erections. After the initial exposure, your doctor may recommend adjusting your dosage. At Olympia Compounding Pharmacy, we work together with the physician and patient to develop a formula that is most effective for the individual patient.

GENERAL TRIMIX DOSAGE GUIDELINES

  • The initial dose should be between .05cc and .2cc.
  • A common initial dose is 0.1cc.
  • Wait for 2-3 days before increasing the dosage.
  • Then, increase by 0.025cc to 0.05cc until a satisfactory dose is reached.
  • The maximum dose of TriMix is 0.50cc.

Phenylephrine is commonly used to reverse the effects of TriMix to prevent an erection lasting too long while the patient is learning their correct dosage. It is also used to reverse the effects of the in-office test dose. Normally .25cc – .60cc of 1mg/ml will satisfactorily reverse the effects.

Selecting the proper injection location is very important. It is also important to know that a 29g or 30g x 1/2″ needle should be used. The needle must be inserted into the penis completely to be sure the needle penetrates the cavernosum. Oftentimes, patients using “auto inject” devices will find the needle does not insert fully, therefore missing the cavernosum, and not getting results from the injection.

Why consider TriMix injections instead of oral medications for ED?

TriMix injections are an alternative to PDE5 Inhibitor tablets for ED (Viagra®, Levitra®, Cialis®) and are most commonly comprised of a mixture of three drugs: phentolamine, papaverine, and alprostadil. TriMix penile injections can be especially useful when patients are unable to take PDE5 Inhibitors because they are taking nitrates, certain beta-blockers, or if they experience severe side effects from the oral medications. Since TriMix is a localized injection (injected into only one part of the body), there are few or no generalized side effects, which is a benefit over oral alternatives that circulate through the bloodstream and can cause headaches, nausea or vomiting (although not very common). Furthermore, injections into the penis, unlike oral medications, trigger an automatic erection in less than 5 minutes.

How does TriMix work?

TriMix is a mixture of three vasodilators—alprostadil, papaverine, and phentolamine—that when injected, work together to cause the corpus cavernosum to relax, expand and fill with blood, creating a powerful erection. Alprostadil relaxes blood vessels, allowing more blood to flow into the penis, while papaverine and phentolamine further enhance this effect by relaxing smooth muscle cells and blocking constriction in the blood vessels. When injected into the penis, TriMix promotes vasodilation and increased blood flow, resulting in a firm and sustained erection for sexual activity.

Does TriMix have side effects?

While TriMix injections are localized treatments and generally cause fewer side effects, some individuals may still experience adverse reactions. These can vary greatly from person to person.

One serious but infrequent side effect is priapism — an erection that lasts for more than four hours. This condition is also a potential risk with other ED medications like Viagra®, Levitra®, and Cialis®. An erection lasting more than four hours is a medical emergency and requires immediate attention.

COMMON SIDE EFFECTS OF TRIMIX MAY INCLUDE:

  • Mild to moderate penile pain or discomfort

Who should not use TriMix?

The administration of TriMix injections is not advised for individuals who have:

  • Allergies to any ingredients present in this medication.
  • Been diagnosed with sickle cell anemia.
  • Blood coagulation disorders.
  • Any condition that increases the risk of priapism (an erection that lasts more than 4 hours and can be painful).
  • Leukemia.
  • Structural abnormalities of the penis.
  • Penile implants.
  • Been explicitly advised by their physician to refrain from engaging in sexual activity.

It’s crucial to inform your healthcare provider of all the medications and substances you are currently taking, including over-the-counter drugs, illicit substances, herbal products and dietary supplements. Certain components in TriMix injections can interact with other medications, potentially rendering them less effective or resulting in adverse reactions.

Are TriMix injections safe?

TriMix injections are a common and well-tolerated method of obtaining an erection used by men with symptoms of ED for roughly 40 years. According to the American Urological Association, injections like TriMix for erectile dysfunction have proven to be a safe non-surgical treatment for men since 1983.[2] Because TriMix is injected into only one part of the body (localized), there are little to no generalized side effects. It’s especially useful for men with severe ED caused by high blood pressure, high cholesterol, smoking, diabetes and obesity. Will TriMix increase size?

Generally, no. TriMix is not designed to make the penis larger. However, TriMix was designed to help men who have symptoms of erectile dysfunction obtain and maintain a full erection for a longer period of time.

Are TriMix injections effective?

According to the American Urologic Association, intracavernous injections are currently the most effective option for non-surgical ED treatment.

Do TriMix injections act quickly?

Unlike oral ED medications, TriMix injections trigger an automatic erection — most in 10 minutes or less. Some men may not begin to see the effect until after sexual foreplay has begun. In the beginning, we recommend using a lower dose to prevent prolonged erections. Your doctor may recommend adjusting your dosage after the initial exposure.

Is TriMix long-lasting?

Erections should last between one and two hours. TriMix injections provide longer and fuller erections than most oral alternatives that can last through the climax, helping men achieve improved sexual health and performance.

When using TriMix or any other ED medication, an erection should never last four or more hours. Erections that last more than four hours are considered a medical emergency. Please immediately contact your local emergency room to be treated if you are experiencing this symptom.

Are TriMix injections easy to self-administer?

The thought of self-administering penile injections can seem daunting for some patients. However, many who’ve tried it find the process manageable and painless. The step-by-step self-injection instructions for TriMix are outlined below:

GETTING SET UP:

  • Start by washing your hands and ensuring the penis is clean.
  • Assemble the necessary items: TriMix bottle, alcohol pad, and syringe (1cc tuberculin syringe with a 29 gauge needle).
  • Keep TriMix chilled by storing it in the refrigerator or placing the bottle in a cup of ice.
  • PREPARING THE SYRINGE:
  • Clean the rubber top of the vial with an alcohol pad.
  • Remove the needle cap, pull the plunger back to the desired dosage mark, filling it with air. Remember to use a new needle and syringe each time.
  • Push the needle through the rubber top and inject the air into the vial.
  • Flip the vial upside down, keeping the needle and syringe inserted, then slowly pull back on the plunger until you reach the required dosage.
  • Tap the syringe side to let air bubbles rise towards the needle, then push out these bubbles before injecting.
  • Pull the needle out from the bottle, and replace the needle cap.
  • SELECTING AND PREPARING THE INJECTION SITE:
  • The appropriate injection sites are at the 9-11 and 1-3 o’clock positions, between the base and mid-portion of the penis. Avoid the midline to prevent injury to the urethra (6 o’clock position) and the penile arteries and nerves (near 12 o’clock). Stay clear of any visible veins or arteries.
  • Hold the penis head and pull it towards your leg side with your index finger and thumb (use the left hand if you’re right-handed). While maintaining this light tension, choose your injection site.
  • Clean the chosen site with an alcohol pad.

INJECTING TRIMIX AND APPLYING COMPRESSION:

  • Push the needle into the skin at a 90-degree angle, penetrating until you hit the hub. You’ll feel slight resistance when the needle is in the correct position within the erectile tissue.
  • Slowly inject the TriMix over approximately 4 seconds. After injection, withdraw the needle and press the site for about 1 minute. If you take aspirin, you might need to press the site for a few minutes to prevent bleeding.
  • Replace the needle cap and dispose of it safely.

Is TriMix affected by alcohol consumption?

Unlike some medications, the effectiveness of TriMix injections is not usually altered by consuming alcohol. While using this medication, patients can drink responsibly and still have the same results as they would have without it.

Are TriMix injections easy to store?

Storing TriMix injections properly is essential to maintain their effectiveness.

REGULAR STORAGE:

  • TriMix injections should ideally be stored in the refrigerator between 32 – 39°F, away from light.
  • The vial currently in use can be kept at normal refrigeration temperature.
  • For short periods, Olympia’s TriMix can be stored at room temperature for convenience while in use. However, it’s preferable to keep the medication refrigerated or frozen whenever possible.
  • TRAVELING AND LONG-TERM STORAGE:
  • When traveling, TriMix can be transported with an ice pack to maintain its temperature.
  • Unused vials should be frozen, kept between 0-31°F. Frozen vials have a beyond-use date of up to one year.
